Embark Therapeutic Services, LLC is committed to protecting the privacy and confidentiality of your mental health information. We are required by law to maintain the privacy of your protected health information and to provide you with this Notice explaining our legal duties and privacy practices.

We may use and share your protected health information to provide, coordinate, or manage your mental health care. This may include consultation with other healthcare providers involved in your care, when clinically appropriate and permitted by law.

We may use or disclose your information for billing, insurance claims, and payment-related activities, if applicable.

For Health Care Operations

We may use or disclose information for practice operations such as quality assurance, supervision, training, licensing, and compliance with legal obligations.

Telehealth Services

Embark Therapeutic Services, LLC provides therapy through HIPAA-compliant telehealth platforms. Clinical information shared during telehealth sessions is protected to the same extent as in-person care.

Please note that while we take reasonable steps to protect your information, electronic communication carries inherent risks.

Uses and Disclosures Requiring Your Authorization

We will obtain your written authorization before using or disclosing your protected health information for purposes not described in this Notice, including:

  • Release of records to third parties not involved in your care

  • Use of information for marketing purposes

  • Sharing psychotherapy notes (with limited legal exceptions)

You may revoke an authorization at any time in writing.

Special Protections for Mental Health Information

Certain mental health information, including psychotherapy notes, substance use treatment records, and trauma-related disclosures, receives additional protection under federal and Oregon law. We comply fully with these heightened privacy standards.

Your Rights Regarding Your Health Information

You have the right to:

  • Request corrections to your records if you believe information is inaccurate

  • Request restrictions on certain uses or disclosures

  • Request confidential communications, such as being contacted at a specific phone number or email

We are required to:
